About
Cosmohealth Pharmacy was founded in 2025 by Jessica Wirawan, an accredited consultant pharmacist who also practises as an aged care, clinical and oncology pharmacist. Jess is especially passionate about the space where aged care and oncology meet, supporting older people whose medicines are often complex.
GPs and aged care facilities engage Jess to carry out Residential Medication Management Reviews (RMMR) for people living in aged care, and Home Medicines Reviews (HMR) for people living at home, following a GP referral. Her focus is making sure every person's medicines stay appropriate, safe and correctly given.
Experience
Jess' background spans oncology, community pharmacy, residential aged care and disability homes. This breadth gives her a practical understanding of complex medicine regimens and the realities of delivering care in different settings.
What a medication review involves
- Eligibility and consent: confirming eligibility and obtaining informed consent from the person or their authorised representative.
- Comprehensive clinical review: medical history, medication chart and profile, progress notes, pathology results, specialist correspondence and discharge summaries, along with prescription, over-the-counter and complementary medicines.
- Person-centred consultation: discussing health conditions, allergies and any concerns the person or their family raise, and making sure their wishes are followed up and respected. For people living with cognitive impairment, Jess liaises with their nurse or carer and their next of kin to help optimise their medicines.
- Clear communication with the GP: a written report with findings and recommendations for the GP and care team, including any requests or concerns raised. Next of kin can also receive a copy of the review summary on request.
- Documentation: uploading reports to the facility's aged care software for record keeping.
- Review tracking: keeping an RMMR register so GPs are reminded when a resident's next review is due.
- Follow-up reviews when needed.
No medication changes are made unless the GP decides to make them.
Funding
Medication reviews are funded by the Australian Government and are AT NO COST to eligible patients and residents.
